How to use this document:
Be sure that you conduct your exit interview in a private, quiet area, free from distractions.
Ask the questions on the document to the exiting staff member, carefully recording their answers.
To ensure that you receive valuable information, some further follow up questions may be required if the staff member provides a vague or generic answer.
Once you have asked all of the questions and recorded all of the answers, type up the notes and store them centrally, restricting access only to management.
Your exit interviews transcripts should be filed together and collated with emphasis on common themes arising, both positives and requiring improvement.
Exit interview feedback should then be discussed and actioned with relevant staff.
